(The General Contractor was called in during a rain storm believing that the leak was a result of a failure of the new roof. As it turnedout, the leak was coming from the incinerator and chimney. The new timer cannot handle this voltage. SEE ATTACHED PROPOSAL. (There are two Leibert climate control units in the Reading Room above the Mechanical Rm. These Leibert units require a fresh air intake. The intake is presently ducted to the fresh air plenum between the filter box for the Archive area climate control and the(SEE 2ND SHEET)(The audio cable removed was too short to reach the new equipment locations and too large to fit 2 cables in the one 1" conduit providedThey are needed to perform daily operations of the facility. (New library shelving has to back tightly to wall. Wiremold switches and wiring needs to be relocated. It also provides better control of the first floor stack lighting. (When the scaffold was erected, we discovered that the window jambs needed recaulking and the steel lintels had deteriorated badly. It isrecommended that the window jambs be recaulked and the lintels be power tool cleaned and cold galvanizing applied. (Provide and install furring and pressure treated plywood over the existing lightweight concrete deck to accept the new slate. Provide new copper flashing closure piece and re-hang existing gutters.(SLATE FASTENERS WILL NOT PENETRATE EXISTING DECK) (Furnish and install two door frames and one door with hardware to interconnect the Bursar's office with support offices. (In order to accommodate future service connections, new 8" and 4"tees,anchors, couplings and blank extension sections are required between existing steam pits A-1 and G-1 of this project. SEE ATTACHED DETAIL DWG. (Furnish and install electronic ballasts as an energy conservation measure. (see attached letter from P. Puhlick to R. McCulloch. (Extend new security system to the newly renovated second floor (Provide and install ice barriers on all nine houses - $200.00 per each house - labor and material. (Provide and install 63 sheets of 1/2" CDX plywood to repair roof deck. $1.40 sq. ft. unit price. (Animal Science needs area used for storage back. Contract calls for University-provided area. (The original contract calls for outside alarms. When activated, thesealarms disturb the whole complex. They should be changed to strobe lights. The price is reasonable. (The contract drawings or specs do not show the meter pads required by the gas company. The price is reasonable. (The contract calls for removal of stop valves at the fin tube radia- tion units. These valves screw onto black iron riser pipes below the existing finish floor. We have found that these valves when removed strip off the threads. (House #40 metal pan roofs are deteriorated. Nails popping out. Re-sheath, doubld bldg. paper required by code for pitch. Reroof porch and rear room (House #36: Provide and install 68 sheets of 1/2" CDX plywood to repair roof deck. 1.40 sq. ft. unit price - $140.00(100 sf) included. (The attached items were not part of the original contract. The using agency requests these items be included in the contract. (Fire pump must be upsized to meet discharge pressure requirements. (Relocate existing condensate pump to allow installation of equipment under BI-D-971 and BI-D-699 air conditioning contracts. (6" x 24' high wall has no reinforcing. Contract calls for 8' x 8' opening and wall must be strengthened and tied together with a bond beam. (Sprinkler pipes and heads have to be added to accommodate H.V.A.C. duct work (BI-D-698). Soffit size must also be increased. (Existing stair & platform do not allow enough room to install diesel firepump fuel tank and controls. Build new 3' x 8' steel platform with removable rail and stair in new location. (Inside 275 gallon fuel tank requires a containment berm per N.F.P.A. (Project designer anticipated another project adding a full Simplex fire alarm system. Controls were to be wired then. Project was not initiated in time. Flow switches and tamper switches should be wired at this time. Replace two mortice locks with horizontal panic exit devices. The doors and hardware originally specified will not work with the removable mullion called for.wHouse #14: Provide & install 35 sheets of 1/2" CDX plywood to repair roof deck. $1.40/sq. ft. unit price. Install wiring for 110V solenoid valve on compressed air line. This solenoid is a safety feature required by program equipment purchased from G.T.R. This solenoid is not shown on any drawing that was received by Facilities. {Repair bituminous at rear of building. Area has settled allowing ponding near building. Probable leak source. Costs to provide engineering for structural steel. Revise design to provide labor and materials to erect new structural steel. Footings would not carry column loads as designed.Delete the relocation of the existing air cooled condenser from the contract and issue a credit to the University. Due to the poor condition of the unit, the shops performed the move and will relocate upon completion. aRemove and cap existing 4" steam main that is in the area of the new contract work. aFurnish and install stairwell cabinet unit heater, piping, controls and insulation. House #22: Provide and install 96 pieces of 1/2" CDX to repair roof deck. $1.40/sq. ft. unit price less 100 sq. ft. KHouse #40: Metal pan roof over entryway leaking. Replaced roof. DProvide new gutters and downspouts. Existing are damaged. This request is to recognize the major funding sources contributing to the construction of these bus shelters. The work required by this C.O. is to furnish and install two(2) bronze plaques as stated in the accompanying documents. JTwo elevators were missed on initial survey at Babbidge Library. Install dead bolts in order to securerooms. Move mag. door holder due to interference. Add one card reader. Add mini controller for office. ?Provide security for second floor PC lab and offices. \Install new sidewalks in front of Engineer Building. SEE ATTACHED. VOID 7/22/93 SEE ATTACHED LIST OF ITEMS THAT MUST BE COMPLETED. Weld on flanges and dispose of existing 550 gallon fuel tank. jWalls are termite eaten and must be replaced. Shoe, walls and plate have been severely eaten.  Field conditions indicate that the two existing isolation valves require replacement; plus additional work at intermediate structure level can be performed in lieu of new access. SEE ATTACHED dated 05/19/93 and Item #3 of original contract document. XUsing galvanized pipe, it could be a cost savings not to paint without damage to product. This change order should be denied. I believe the credit is not enough. The schedule of values for finishes is $14,100.00. The contractor was advised to break down the finishes shcedule and re-submit new credit. He never replied. POperations requested a new hydrant be installed near the auditorium. Dick Brand requested a new 6" gate be installed which was not shown on drawing. This is a code compliance under NFPA 24 4.2-1. tInstall a new fire hydrant off of North Eagleville Road. This is a code compliance under NFPA 24 4-2.1. When installing the new water line, pipe had to be installed under an old and severely damaged 10" clay sewer pipe. It was decided that the best thing to do was to install a new sewer line to the nearest manhole.  At the intersection of Hillside Rd. and Auditorium Rd., the contractor had to relocate the water line due to obstruction of a large steam pit. Also, there was an existing steam return leak which had to be located and repaired which was not in contract. FOvertime in Purchasing Dept. only as per attached agreement. Furnish and install a wet tap valve to service Central Warehouse in lieu of shutting down water service to various buildings. SEE ATTACHED Original location of H.C. door was changed as requested and a vision panel was omitted in a door but desired by client. 1Unsuitable soil was encountered at two of the three sites along with an excessively high water table, requiring removal of soil and dewatering. Additional backfill was required and stone fill to allow dewatering. This was unforeseen because the contract did not have any boring data. Haddam as builts show an existing 3" drain. 2" is actually there. Remove 2" and install 3" drain for lav. per code. House 23: Provide and install 104 sheets of 1/2" CDX to repair roof deck. $1.40/sq. ft. unit price less 100 sq. ft. House 25: Provide and install 58 sheets of 1/2" CDX to repair roof deck. $1.40 sq. ft. unit price less 100 sq. ft. House 26: Provide and install 32 sheets of 1/2" CDX to repair roof deck. $1.40 sq. ft. unit price less 100 sq. ft. House 29: Provide and install 96 sheets of 1/2" CDX to repair roof deck. $1.40 sq. ft. unit price less 100 sq. ft. R9 dial up modems are not practical due to existing telephone line configuration. Intrusion without signaling could occur. Change to R7 modems. Asbestos found in basement floor tile and mastic. Removed by University contract abatement contractor. Add 1,000 sq. ft. to redo areas not in original scope due to abatement. Two existing doors do not meet clearance codes. Remove doors, frames and a portion of wall. Provide and install new doors and hardware. Delete all work on plaza decks. In order to cover the unexpected costs that have to be covered by this project allotment, the least critical items have to be deleted. The added items will be on C.O. #'S 2, 3 and 4. (SEE ATTACHED PROPOSAL OPTION #1). nRemove all asbestos flashings and related materials as described in attached quotation. This originally was planned and specified to be done on contract award by EH&S under separate funding. Having two separate contractors trying to meet diverse objectives creates an almost unworkable situation in coordination of work and adds appreciably to total cost to the State. Facilities was notified after the initial job meeting that there was no longer any funding in the EH&S account to do this type of work. The cost of this removal will be covered by credit from C.O.#1-SEE ATTACHED PROPOSAL. In accordance with specifications, pull tests were performed to determine if the full lightweight concrete, etc. had adequate strength to meet the requirements of the manufacturer of the single ply unadhered roof that was specified. All test results failed to meet the manufacturer's requirements which would automatically void any guarantee or roof bond. It was decided that the least expensive way to correct this problem was to use a fully adhered Owens-Corning built-up roof having a 20 yr bond and still would provide the same or better insulating characteristics. This would have a gravel surface similar to the original roof that lasted over 20 yrs. Other methods of resolving this problem were analyzed and all were move expensive and would at best have had a 15 yr guarantee. This does not change any other conditions of the original specifications. SEE ATTACHED PROPOSAL. The Mansfield Training School was turned over to the University by DPW after the RPF was prepared. The survey is needed to determine an estimate on the funds required to make the buildings usable. rLocated at the east stairs, the limestone repairs were not included in this contract. (SEE ATTACHED) HADDAM: Water leaking through foundation wall and puddling on new floor in two locations. 1. Pave 5'x10' area to carry water away from foundation wall. 2. Excavata at leaking area, around soil pipe and patch with hydraulic cement. When excavating, we encountered an old concrete foundation. The piping could not be installed. Contractor had to remove a 52' long, 14" thick, 4' high concrete wall plus footings and concrete floor all with re-bars. 4Ledge removal to install steam pit H-1A. #Various work: Elevation change required to install steam and condensate between manholes C-1 and H-1A; cleanup delay and repair of 6" ductile water main. Clean caved-in dirt from new piping due to water main break. Extend pipe on northwest side of Eagleville Rd. to avert a 10" transite forced main. Per Ron Nichols request, install a thrust block on elevation change between manholes G-1 and A-1. Cut 5' of pipe off 8" steam main to Pit A-1. Remove and reinstall 4" ductile iron pipe between manholes C-1 and H-1A. Repair and reline gutters at Whitney and Sprague buildings. The gutters were worn thin with holes 1/2" dia. or greater at one to two foot intervals all along its length. The plans indicate to reuse existing gutters. To replace would be too costly and the delay would extend the project past the allowable completion date. The substructure is sound. Minor repairs and relining is both feasable and economical. Reflash areas at base of penthouse found to be leaking because of original building design. This condition was not evident until old roof was removed. Most extensive leakage was in wall louvre sections. SEE ATTACHED PROPOSALS NEEDED TO ACCOMPLISH THIS WORK. eAt Holcomb bldg., the gutters and supporting members are shot. Total replacement of gutters and associated flashings must be done. Additional support for the existing support members and gutter/facia repair is necessary. The cost is reasonable. The plans show existing gutters to remain and be reused. Downspouts are not included. The specifications stated to furnish white roofing. Mfr's literature stated colors were standard but upcharge was not known at design stage. Since roof is seen from a large adjacent residence hall complex, a colored membrane is desirable. Also, anodized aluminum facia to match existing in lieu of kynar finish requires small upcharge. Since both of these items are aesthetic concerns, the architect decides what is required. wOne room did not get the required cooling. Change in room arrangement shut off cooling from adjacent room. URepoint interior stone foundation per unit price. Joints deteriorated. The top rail piece needs to be replaced. The contract specified that the existing railings be reused. There was no repair required by spec. The top of the rail was a spot welded half round member to square stock. Water had infiltrated between these members and rust build up caused them to bow and separate. A new top piece will be installed. The cost is reasonable. The contract called for replacing existing snow guards. The existing layout was missing a vast number of guards and did not address primary entrance areas. The new layout addressed all roof aresa and should eliminate any concerns. The price is reasonable. QExcavate pump water and repair 2" water main running to building #29. Additional core-boring through existing concrete roof shown on the typical wall section on sheet A1 to be 4" thick, which actually is 8" thick. Relocate existing catch basin which was buried by bank erosion. Regrading required to raise basin. Time extension is approved. lNew steam, condensate and cold water lines from Engineering 1 to Puerto Rican Cultural Center. At Parking Lot "W", as per approval of Engineer, the Contractor raised a double catch basin, installed a new catch basis and some underdrain work. QWe used existing curbing instead of new as per bid credit Item #4A. When existing islands were opened up, it was found that no loam had been removed when constructed. The contractor had to remove unsuitable soil and replace with bank-run gravel. Asbestos found on pipes above ceiling. Sealed ceiling system concealed conditions. Price coordinated with E.H.& S. Days must be rejected. 7Existing stair nosing is in good condition. Concrete is spalling from beam and presents a hazard. Chip off concrete and spray on fireproofing. Concealed by non-accessible ceiling. Disallow time extension. Duct work obstructs new soffits; cannot be hung as designed. Non-accessible ceiling prevent pre-bid inspection. Disallow time extension. A/V package was as specified for Gentry. Must be modified to accommodate different lecture hall configuration. Disallow time extension. Specified amp is too small for the number of speakers and ceiling height. Increase from 60 to 100 watts. Disallow time extension. Reinsulate ducts and pipes above ceiling. Insulation was removed for asbestos abatement. Disallow time extension. Removal of asbestos was not included as part of this contract. Asbestos test results showed ACM at several areas including flashings and patch areas. fBrick walls in various locations need repair. This work was not included in the contract. sThe Design Engineer added new lock sets to five additional existing doors which were not in contract. OExpand driveway entrance behind Life Science Bldg. per Drawing A-1. CAfter the project work was in progress, it was discovered that an approximately 200' section of sidewalk was not included in contract. Engineering field design was necessary, two light poles had to be relocated, changes in curbing installaton and 16' x 200' blacktop had to be removed and re-installed. Provide Gateway 4DX266E system w/340 MB drive, including HP Laser Jet IV printer and US Robotic 9600 modems at $436/per system added cost. Specified model has now been replaced by this more versatile system. The existing wood nailer along the perimeter west s.w. side and gypsum deck was rotted or otherwise deteriorated, requiring total replacement. The spec only required repair of existing deck. The cost is for this section only and includes unit price for deck repair. This is for repair of existing gypsum deck per unit price required in bid document. This is only for the area patched August 20, 1993. The specs call for fasteners at 18" o.c. The pull-out test performed by Olumpic Fastener Systems for the roof system manufacturer require anchors at 12" o.c. This change results in an additional cost per the attached documents. Modify controls to utilize the steam coil for dehumidification. Slow fan down and adjust fresh air intake. Rebalance the system. @This is a change to existing hardware - handicap trim. When curbing islands were being installed, it was discovered that the light bases and poles were too close to the road. It was decided to move nine(9) each back at a unit cost of $1,000.00 each. {SEE ATTACHED PROPOSAL. Waterproof Mechanical Penthouse floor. Although all roofs were made weathertight, there is periodic flooding of the Mechanical Room from various pieces of equipment. This leaks into the rooms below just as though it were a roof leak. This work was not in original request but deleted in design to keep project within funding limits. $The floor covering limits and types were changed as a result of available colors to match adjacent materials chosen by the department and as directed by the architect. The extent of rubber floor increased and numerous feature strips divide the areas where new and old meet. Due to electrical and telephone duct banks and other utilities, the contractor was unable to follow the pipe direction as designed. This change order is for labor and material of the new design. Asbestos has been found in the perimeter flashings of the built up roof over the dining hall. The specs do not cover removal. The flashings must be removed in order to replace the roof. Section 02405 of the Contract Specification Catch Basis Top Replacements calls for replacement of catch basin tops as shown on contract drawings. The contract drawings show 17 each and existing there are 23 each. The gypsum roof deck required patching. The entire deck was in poor condition. The patched areas varied in length, width and depth. The unit price is per the bid documents. GThe entire gypsum deck was in disrepair. Some sections needed total replacement involving removal of deteriorated gypsum and formboard, reinstalling new formboard, reinforcing wire and repouring gypsum in 2" to 3" thickness. This was not covered by specifications and was not evident when viewed from below. $The wood blocking along the perimeter was deteriorated in several areas beyond repair. The specs did not cover replacement. The new nailers specified could not overlap the deteriorated areas. The facia also needed replacement due to vegitation growing around and over it. ~Furnish and install footing drains at the Starr Building around window well areas and delete drain at Hosmer Hall. Furnish and install 160 L.F. of new 10" drain line to further separate system with the new Law Library per request of Peter Puhlick. lDelete all bituminous paving at the entrance road and circle in the area of the new Law Library. A.C. units draw too much current for the existing breakers. This change will provide proper breakers and necessary wire to permit satisfactory operation.
